3. Any special instructions to deliver the Life Pulse to your department

General Information
A purchase order must be provided within 24 hours of the delivery of your rental (excluding weekends). Please contact Bunnell's Customer Service Department at 800.800.4358 ext. 6 or fax purchase orders to 801.467.0867.

Please remember there are no minimum charges for rentals but charges will accumulate until the rental equipment is received at Bunnell Incorporated, Salt Lake City, UT. Therefore, be sure to return your rental as soon as you are done with it.
