How to Terminate Your Rental
TERMINATING YOUR RENTALFor additional information please contact our Customer Service Department at 800.800.4358 ext. 6 Monday through Friday 8:00 a.m. to 5:00 p.m. MT.
- The Life Pulse must be returned in its original shipping case.
- Contact Bunnell's Customer Service Department at 800.800.4358 ext. 6 for a return authorization (RA) number.
- Write the RA# on the enclosed return shipping label and place the label on top of the shipping case.
- If the shipping label has been lost, the rental must be returned to:

Salt Lake City, UT 84115.- Please remember there are no minimum charges for your rental. However, charges will continue to accumulate until the rental equipment is received at Bunnell Incorporated.
- To avoid additional rental charges, Next Day Air for the return of your rental is recommended.
- Included with the rental is a Life Pulse Operator's Manual. If it is not returned with the rental equipment you will be billed an additional $75 on your rental invoice.
- Rentals are invoiced a minimium of once per month or when the equipment is received.
