How to Rent a Life Pulse®
* NOTE: LifePulse Model 204 rentals are only available from the Salt Lake City location. Plan 204 rentals accordingly.
During normal business hours 8:00 a.m. to 5:00 p.m. MST
Call 800.800.4358, press 5. A Bunnell representative will need:
1. Your name and telephone number
2. Hospital name and address
3. Any special instructions to deliver the Life Pulse® to your hospital
After business hours
Call 800.800.4358. Our answering service will contact our on-call specialist. He will need the following information:
1. Your name and telephone number
2. Hospital name and address
3. Any special instructions to deliver the Life Pulse® to your department
General Information
A purchase order must be provided within 24 hours of the delivery of your rental (excluding weekends). Please contact Bunnell's Customer Service Department at 800.800.4358 ext. 6 or fax purchase orders to 801.467.0867.
Please remember there are no minimum charges for rentals but charges will accumulate until the rental equipment is received at Bunnell Incorporated, Salt Lake City, UT. Therefore, be sure to return your rental as soon as you determine that you no longer need the Life Pulse®.

How to Terminate a Life Pulse® Rental
During normal business hours 8:00 a.m. to 5:00 p.m. MST Call 800.800.4358, press 5. A Bunnell representative will need:
1. Your name and telephone number
2. Hospital name and address
3. Serial # of device(s) being returned
4. To provide you with an RGA#
After business hours Call 800.800.4358. Our answering service will contact our on-call specialist. He will need the following information:

2. Hospital name and address
3. Serial # of device(s) being returned
